1. Completed & Signed Registration form (Includes Code of Conduct) (mandatory for players to be eligible to play).
   
   
2. Photocopy of your child's birth certificate if new to AMHA.
   
   
3. Outstanding balances from the 2008 season to be resolved before 2009 registration is accepted.



4. 2 Post Dated Cheques, August 1, 2009 & September 30, 2009.

   1. Applicants 6 years and up (Born 2003-1992). Player Registration $420:
      
      Registration received after June 15, 2009 : $450.00 (waiting list in effect)
      
      • Post-dated cheque for 50% of the total registration amount, dated August 1, 2009
      • Post-dated cheque for 50% of the total registration amount, dated Sept 30th, 2009
      
      
   2. 4/5 year old applicants (Born in 2004/2005) Bumble Bee Registration $315:
      
      
      • Post-dated cheque for 50% of the total registration amount, dated August 1, 2009
      • Post-dated cheque for 50% of the total registration amount, dated September 30, 2009
   
   
   
6. Mail to, or drop off complete registration package and payment to AMHA PO Box or drop box outside of AMHA Office in Arena "A"

Additional Notes:

1. Registration fees covers ice times ending February 15th. AMHA will provide additional ice time based upon a team's advancement throughout the playoffs.
2. All registration forms will be numbered based upon order of acceptance. Incomplete registration forms will not be numbered and returned to the family. In the unfortunate event that Arnprior Minor Hockey is unable to accept all applicants, registration will be based upon the sequential order or the registration forms.
